Internal Tooth Whitening: Case #17 — Restoring a Discolored Front Tooth

A single dark front tooth can undo an otherwise healthy smile. In Case #17, Dr. Cvetan Mechev, DDS of La Mesa Dental Center used internal tooth whitening — sometimes called internal bleaching — to restore a patient’s discolored front tooth without a crown or veneer.

The Problem: A Dark, Discolored Front Tooth

This patient’s front tooth had darkened from the inside, a common result of a previous root canal treatment. When a tooth loses its blood supply and nerve, the remaining tissue inside the tooth can slowly stain the surrounding structure, turning the tooth gray, brown, or yellow-brown compared to the teeth next to it.

Because the discoloration originates inside the tooth, regular whitening trays, strips, or in-office external whitening cannot reach it. The tooth needed a treatment that works from the inside out.

The Solution: Internal Tooth Whitening

Dr. Mechev recommended internal tooth whitening, a conservative, minimally invasive procedure that lightens a nonvital (root canal–treated) tooth from within, rather than covering the discoloration with a crown or veneer.

During the procedure:

• Dr. Mechev accessed the inside of the tooth through the existing root canal opening
• The area was cleaned and sealed to protect the root and surrounding gum tissue
• A whitening agent was placed inside the tooth’s internal chamber
• The tooth was temporarily sealed while the whitening agent worked from the inside out
• Once the desired shade was reached, the access opening was permanently sealed and restored

This approach preserved the patient’s natural tooth structure and avoided the cost and additional preparation required for a crown or veneer.

The Result

The before-and-after photos show the transformation: a front tooth that once stood out as noticeably darker than its neighbors now blends naturally with the rest of the patient’s smile. The result is a conservative, cost-effective fix that addressed the discoloration at its source.
